Goal 1: Maintain and sustain implementation of LiveWell Alamosa
- Photovoice.
- Maintain and build on working partner relationships and communication.
Goal 2: Implement policies and programs that ensure healthy eating and active living in the school setting
- Work to implement wellness policy through School Wellness Committees at ASD and SDC Districts using the Coordinated School Health Model 2008.
- Work with Food Service Directors to implement Farm to School practices at ASD and SDC ensuring that all foods and beverages in school are healthy 2008 through 2009.
- Implement use of Take 10® 2008, and increase adherence in use of Take 10® by elementary teachers in Alamosa County in 2009.
- Support nutrition and physical activity into more curricula through including teaching greenhouses at SDC and ASD and have school facilities available to families outside of regular school hours at both ASD and SDC districts in 2009.
Goal 3: Increase wellness in worksites
- Mini-grants to expand VWHS Cubicle gyms and paid employee break time to use them throughout the organization.
- Encourage adoption of worksite wellness policies by working with Alamosa Chamber and Small Business Development Center to offer Worksite wellness resources and presentations in 2009.
Goal 4: Improve practice-based assessment and management of overweight and obese patients
- Increase knowledge and use of referral resources such as Pro Fit Weight Loss Program offered by the SLV Regional Medical Center and the free Colorado Fit Now Weight Loss phone counseling program for patients. (2008, 2009)
- Provide education about and increase use of Colorado Clinical Guidelines Collaborative Adult Obesity Guidelines in the clinical setting. (2007, 2008, 2009)
- Become trained as a lay leader of Stanford University Healthier Living Course 2008 and teach two Healthier Living Classes in the community in 2008 and 2009 and become trained in the Spanish self-management course Tomando Control and teach one Spanish class in 2009.
- Learn about and implement pediatric guidelines for obesity at VWHS, SLVPS and Rock Creek Practices using WIC practices. (2008, 2009)
Goal 5: Increase physical activity and healthy eating in the community of Alamosa
- Implement baby-friendly policies at San Luis Valley RMCW.
- Support existing and set up a minimum of two more lactation rooms in the community. (2007, 2008, 2009)Offer breastfeeding education and support to medical staff, lay leaders and parents. (2007, 2008, 2009)Learn about Smart Meal SeaI 2008 and implement Smart Meal Seal in Five local restaurants including the two school districts 2009Provide nutrition education in a variety of settings, including grocery stores, restaurants, farmers markets, cooking shows. (2007, 2008, 2009)Build greenhouse and garden capacity in the San Luis Valley supporting three existing sites and five new sites, and continue to get vegetables into the food bank and La Puente Homeless shelter while offering these participants opportunities to work in the gardens and greenhouses. - Improve opportunities for Older Adults to eat better and safely exercise expanding throughout SLV in 2009. (2007, 2008, 2009)
- Explore relationship with Humane Leagues and walking programs to offer a new venue for physical activity and expand programs that educate or implement physical activity and nutrition at the Alamosa Recreation Center and Boys and Girls Club. (2007, 2008, 2009)
- Educate community members and decision makers about the importance of active community environments, conduct an on the ground walkability/bikability audit of Alamosa towards implementing Safe Routes to School and Complete Streets, and begin work to create a park with playground equipment in Mosca, Colorado. - Develop relationships with Early Childhood Council of SLV 2008.
- Conduct Child Care Champions Best Practices Assessments at a minimum of five licensed centers in 2009.Continue to support and enhance the practice of breastfeeding by mothers and babies in the San Luis Valley by implementing baby-friendly policies and practices, and providing education, support, and follow-up.
